How to create a new form and start collecting new records using the Forms Application

What is the Forms Application?

Populate your online database while enforcing data health and structure in a secure manner with the Forms Application built on the Pitchly Platform. 

Forms is useful in requesting, collecting, and evaluating data - ultimately strengthening your database. To get started, ensure the forms app is installed in the database of choice and that an admin has granted forms permission.

Creating a Form

New Form

Navigate to Forms within the main database view.



Click on the plus icon so the "create a new form" box appears and asks you to enter a name. Enter a name to identify the form and click create.


Adding Fields

The fields mirror the database fields exactly. Each field you see available in the form was created in the database. By default, all fields are selected to be included in the form. Unselect any fields that you do not wish to include in the form. Or to unselect all, click the checkbox next to the search bar.



Selected fields will appear at the top of the list under Shown, while fields not selected will remain under the list labeled Hidden.

Arranging Fields

To rearrange the order in which the fields appear in the form, simply drag and drop the fields under the Shown list. The view will automatically update on the form in real-time. 


Once you are satisfied with the selected fields and the order in which they appear select Save Changes at the bottom of the screen.

Adding some Flare to a Form

Within the Design view of the form, you have the ability to update the form title, provide instructions users will see when completing the form, and change the background color of the form.


Sharing a Form

Public Link

There are multiple ways to share the form. The public link to the form can be sent directly to a recipient or placed in a globally accessible location, such as the company intranet site. Simply copy and paste the link that is generated in the form.

Forms can also be shared by entering recipient email addresses. This will generate an email notification to each user inbox.


You have the option to indicate the date and time a form should be sent and the ability to set a future date when the form should expire. Additionally, reminder emails can be scheduled to go to recipients at set intervals in the future.


If responses should be limited to one response per recipient, check the option to indicate such.
